PROBLEM: A seizure is a challenging situation for children with epilepsy. Little is known regarding the experience of children who perceive in advance that they are about to have a seizure. METHODS: From September 2020 to February 2021, we invited children with focal epilepsies aged 6-18 years to participate in a semi-structured interview. RESULTS: Of 52 children with focal epilepsies, 22 (42 %) said they perceive in advance that they are about to experience a seizure [11 with self-limited epilepsy with centro-temporal spikes (SELECTs), 11 with other focal epilepsies]. All 22/22 (100 %) children described physical symptoms such as headache or a numb feeling in one half of the body. Of those children, 17/22 (77 %) stated they try to do something about the seizure. Those strategies were perceived as helpful by 0/11 (0 %) children with SELECTs and 9/11 (86 %) children with other focal epilepsies (p < 0.001). Of the children with SELECTs 5/11 (45 %), and of those with other focal epilepsies 9/11 (86 %) stated they would like to know in the morning if they are to experience a seizure that day (n.s.). CONCLUSION: Children who perceive in advance that they are about to have a seizure are well able to describe their experience. Most children take measures to manage their seizures. Those measures were regarded as helpful by most children with other focal epilepsies, but by no child with SELECTs. Larger studies are necessary to determine the factors contributing to the child's perception as well as the nature of the support that they require.

AIM: Chronic conditions can influence the situation of healthy siblings of affected children. We investigated the opinion of the affected child about the situation of their healthy sibling and the sibling relationship. METHODS: We performed a semi-structured interview with epilepsy or asthma patients aged 6-18 years and asked them to draw a picture: "Epilepsy/Asthma, my siblings and me." RESULTS: Of the 58 children with epilepsy, 67%, and of the 40 children with asthma, 60% thought that their siblings were worried about the condition. Among other aspects, they addressed helplessness during a seizure. Of the children with epilepsy, 83% and of those with asthma, 95% assumed that their siblings were not disadvantaged because of the participant's condition. Of the patients with epilepsy, 91% and of those with asthma, 93% thought that the sibling relationship would not be different without the condition. Of the participants with epilepsy, 86% and of those with asthma, 93% drew a picture; 30% with epilepsy and 14% with asthma visualised an interaction with their siblings in context of a seizure or episode of dyspnoea. CONCLUSION: According to the affected children, the condition worries their siblings but does not affect the siblings' lives or the sibling relationship.

Strategies of Emotion Regulation in Adolescents with Depression The present study investigated differences between adolescents with a depressive disorder and control subjects in their adaptive und maladaptive regulation of negative emotions. We assessed emotion regulation strategies using the German self-report questionnaire FEEL-KJ in a sample of adolescents with depression (n = 57) and controls (n = 57). We found that adolescents with depression reported using less adaptive strategies and more maladaptive strategies for dealing with the negative emotions anxiety, sadness and anger than control subjects. Our findings indicate that adolescents with depression showed difficulties with regard to different negative emotions that should be considered in intervention.

Outdoor recreation is one of many important benefits provided by public lands. Data on recreational use are critical for informing management of recreation resources, however, managers often lack actionable information on visitor use for large protected areas that lack controlled access points. The purpose of this study is to explore the potential for social media data (e.g., geotagged images shared on Flickr and trip reports shared on a hiking forum) to provide land managers with useful measures of recreational use to dispersed areas, and to provide lessons learned from comparing several more traditional counting methods. First, we measure daily and monthly visitation rates to individual trails within the Mount Baker-Snoqualmie National Forest (MBSNF) in western Washington. At 15 trailheads, we compare counts of hikers from infrared sensors, timelapse cameras, and manual on-site counts, to counts based on the number of shared geotagged images and trip reports from those locations. Second, we measure visitation rates to each National Forest System (NFS) unit across the US and compare annual measurements derived from the number of geotagged images to estimates from the US Forest Service National Visitor Use Monitoring Program. At both the NFS unit and the individual-trail scales, we found strong correlations between traditional measures of recreational use and measures based on user-generated content shared on the internet. For national forests in every region of the country, correlations between official Forest Service statistics and geotagged images ranged between 55% and 95%. For individual trails within the MBSNF, monthly visitor counts from on-site measurements were strongly correlated with counts from geotagged images (79%) and trip reports (91%). The convenient, cost-efficient and timely nature of collecting and analyzing user-generated data could allow land managers to monitor use over different seasons of the year and at sites and scales never previously monitored, contributing to a more comprehensive understanding of recreational use patterns and values.

Social media and text messaging show promise as public health interventions, but little evaluation of implementation exists. The B'more Healthy Communities for Kids (BHCK) was a multilevel, multicomponent (wholesalers, food stores, recreation centers) childhood obesity prevention trial that included social media and text-messaging components. The BHCK was implemented in 28 low-income areas of Baltimore City, Maryland, in 2 waves. The texting intervention targeted 241 low-income African American caregivers (of 283), who received 3 texts/week reinforcing key messages, providing nutrition information, and weekly goals. Regular posting on social media platforms (Facebook, Instagram, Twitter) targeted community members and local stakeholders. High implementation standards were set a priori (57 for social media, 11 for texting), with low implementation defined as <50%, medium as 50% to 99%, high as ≥100% of the high standard for each measure. Reach, dose delivered, and fidelity were assessed via web-based analytic tools. Between waves, social media implementation improved from low-moderate to high reach, dose delivered, and fidelity. Text messaging increased from moderate to high in reach and dose delivered, fidelity decreased from high to moderate. Data were used to monitor and revise the BHCK intervention throughout implementation. Our model for evaluating text messaging-based and social media-based interventions may be applicable to other settings.

PURPOSE: To conduct a descriptive analysis of the results from the Functional Assessment of Cancer Therapy-General (FACT-G) quality of life (QOL) questionnaire, describe the outcomes from the FACT-G to drive treatment recommendations within the breast survivorship clinic and to quantify the severity of QOL issues experienced. METHODS: A retrospective analysis utilizing medical records of participants in a breast cancer survivorship clinic. Measurement data included demographics and FACT-G results. Descriptive analysis of demographics and trends in referral recommendations and FACT-G scores was completed. RESULTS: All 30 participants were females diagnosed with breast cancer of various stages, ages 28 to 81 years. Approximately 1.5 years elapsed between cancer diagnosis and completion of the FACT-G. Participants received surgery (100%), radiation (76%), and chemotherapy and/or hormonal therapy (43%). Results demonstrated that participants reported having a lack of energy (24%) and were bothered by side effects of their treatment (20%). The greatest impact on functional well-being was difficulty sleeping (50%). LIMITATIONS: Decreased ability to generalize the data to breast cancer survivors due to small sample size from one institution and potential referral bias. CONCLUSIONS: Cancer survivors experience QOL issues throughout the continuum of their care, which can result in long-term effects on their physical, functional, social and emotional well-being. QOL is a major focus for cancer survivors and many times determines a survivor's healthcare decisions. QOL measurements can be utilized at multiple points during survivorship to identify the need for referrals and to guide interventions.

The present study investigated differences between adolescents with ADHD and control subjects in their adaptive und maladaptive regulation of negative emotions. We assessed emotion regulation strategies using the German self-report questionnaire FEEL-KJ in a sample of adolescents (between 11 and 18 years) with ADHD (disturbance of activity, impulsivity and attention: n = 32, hyperkinetic conduct disorder: n = 26) and controls (n = 58). We found that adolescents with ADHD reported using less adaptive strategies for dealing with negative emotions than control subjects. No effects were found for maladaptive emotion regulation strategies for anger, fear and sadness. Our findings indicate that adolescents with ADHD should be encouraged in the development of adaptive emotion regulation.

Radon ((222)Rn) and progenies emanate from soil and building components and can create an indoor air quality hazard. In this study, nine concrete constituents, including the supplementary cementitious materials (SCMs) fly ash and metakaolin, were used to create eleven different concrete mixtures. We investigated the effect of constituent radium specific activity, radon effective activity and emanation fraction on the concrete emanation fraction and the radon exhalation rate. Given the serious health effects associated with radionuclide exposure, experimental results were coupled with Monte Carlo simulations to demonstrate predictive differences in the indoor radon concentration due to concrete mixture design. The results from this study show that, on average, fly ash constituents possessed radium specific activities ranging from 100 Bq/kg to 200 Bq/kg and emanation fractions ranging from 1.1% to 2.5%. The lowest emitting concrete mixture containing fly ash resulted in a 3.4% reduction in the concrete emanation fraction, owing to the relatively low emanation that exists when fly ash is part of concrete. On average, the metakaolin constituents contained radium specific activities ranging from 67 Bq/kg to 600 Bq/kg and emanation fractions ranging from 8.4% to 15.5%, and changed the total concrete emanation fraction by roughly ±5% relative to control samples. The results from this study suggest that SCMs can reduce indoor radon exposure from concrete, contingent upon SCM radionucleotide content and emanation fraction. Lastly, the experimental results provide SCM-specific concrete emanation fractions for indoor radon exposure modeling.

We have used small-angle light-scattering (SALS), microscopy, and measurements to study structural changes produced in unbuffered agarose gels as ions migrate under applied electric fields (3-20 V/cm). Anisotropic, bowtielike, light-scattering patterns were observed, whose development occurred more quickly at higher fields. The horizontal lobes were more pronounced at higher polymer concentration. Analysis of the SALS data with a simple model of scattering from anisotropic rods in an electric field is consistent with anisotropic rodlike domains on the order of 10-15 microm in length, which align perpendicular to the electric field. The anisotropic domains in the gel reach almost the same level of orientation, regardless of the field strength. Microscope imaging revealed anisotropic domains on the same length scale, also aligned perpendicular to the field. Profiles of pH variation across the gel, measured by video photography, indicate that the anisotropic patterns appear when the H+ and OH- ions, migrating in opposite directions, meet. Calculations of pH profiles using a model based on electrodiffusion reproduce several features of measured pH profiles, including the power-law dependence on the electric field of the time at which the oppositely charged fronts meet. Ions migrating from both ends of the gel produce pH changes that are correlated with macroscopic shrinking and orientation of the gel.
